Terms of booking

By submitting your Booking Request, you acknowledge that personal medical and health information and all other personal data that you have and may provide will be collected, processed and stored by the National Migraine Centre and its agents for the purposes of providing services and healthcare treatments. Your data will be accessible by staff, suppliers and volunteers of the National Migraine Centre, and to clinicians and other contractors, in order to enable the triage, administration and delivery of clinical services. Associated third parties who may have access to and processing rights to your data include, but are not limited to, pharmacists, IT providers, organisations involved in the processing of prescriptions, suppliers of medical devices and patient management software suppliers. Data may be shared with your GP, hospital consultant or other medical specialists. For more information, please see our Privacy Policy, which is available on our website. The information you have supplied in this form will be sent by email to both the National Migraine Centre and also the email address you have supplied.

Particular treatments may not be suitable for you. Please review our website for guidance before proceeding and seek professional or specialist advice where applicable. You will be offered a consultation with a clinician; we do not guarantee a procedure or prescription in advance, irrespective of the appointment type you may have chosen, since the clinician may determine a particular treatment is unsuitable for you, you may not meet eligibility criteria or you may withdraw consent. If a Botox or nerve (GON) block procedure is cancelled in those circumstances, you will be reimbursed the money that you have paid for that procedure, less a consultation fee for the doctor’s time (equivalent to the current fee for a first private appointment, which can be found on our website). If you have requested a Botox or nerve block procedure, you confirm that you have been diagnosed with migraine or cluster headache by a GP or consultant neurologist. If a prescription is issued, you will need to pay a pharmacy for the cost of your medication (private prescriptions can cost much more than NHS prescription charges). You agree to discuss with your clinician potential side effects and to share with them details of any current medications or other relevant information that may help inform your treatment plan.

By submitting your Booking Request, you confirm that this was the information supplied was submitted by the patient themselves, except where the patient is under 16 (in which case it may be completed and submitted by a parent or legal guardian) or where the patient lacks the mental capacity to make informed healthcare decisions. In cases where the patient lacks mental capacity, you confirm that you have received written agreement from the National Migraine Centre in advance that this form will be completed on behalf of the patient.

By submitting this Booking Request you agree that you will be within the UK, Isle of Man, Guernsey or Jersey at the time of your appointment and will let us know if this changes – if you are temporarily out of these territories on the date of your appointment, we will endeavour to provide an alternative date on your return. By submitting this Booking Request you also confirm that you are registered with a UK general practitioner.

The National Migraine Centre puts people in touch with qualified medical professionals who can provide advice and medical treatment. These clinicians are not employees of the National Migraine Centre. These clinicians have no authority to act for or represent the National Migraine Centre in any way or otherwise be deemed an agent of the National Migraine Centre, and nothing herein shall be construed as creating any other relationship between the clinicians and the National Migraine Centre. These clinicians do not have the right or authority to assume, create or incur any liability or any obligation of any kind, express or implied, against, in the name of, or on behalf of the National Migraine Centre.

The charity does not provide medical advice, consultations, procedures or any other medical services and is not responsible for the actions or advice provided by those medical professionals to whom we may direct enquirers. Liability for the information, advice and treatment given by those medical professionals rests solely with the clinicians themselves.

By submitting your Booking Request, you consent to receive healthcare advice and treatment delivered by clinicians appointed by the National Migraine Centre. Additional consent may be taken at a later stage where medications are prescribed, or procedures are recommended.

At any medical appointment we offer you, you may be asked to prove your identity by showing an identity document, such as a passport or driving licence. Details of which documents are acceptable for the purposes of proving identity may be shared with you in advance of your appointment. If you fail to show one of these approved documents at your appointment, despite having been informed of the need to do so in advance, your appointment may be cancelled, and the National Migraine Centre will not have any obligation to reimbursed you for any fees paid.

If you fail to attend an appointment, or cancel after the cancellation deadline (as shared in our pre-appointment emails), you will be liable to a cancellation charge.
