Consent forms

If you’ve booked a procedure or have been prescribed medication, you’ll need to complete and return your consent form before we can go ahead. Choose the form that’s right for you below.

Procedures

For Botox or GON block procedures, you can return your consent form at any time after your consultation is confirmed by email. But we must receive your completed form at least three working days prior to your procedure.

If we have not received your form three days in advance, your appointment may be cancelled without a refund.

If your procedure doesn’t go ahead because a clinician determines the procedure is not appropriate for you, we will reimburse your costs, less the clinician’s fees.

Prescriptions

You’ll usually be asked by us to fill in a consent form after your appointment. We’ll then need to receive your completed consent form before we can process your prescription.
